Radiation Oncology:  
What are types of radiation therapy: There are two types of radiation therapy. 

1. Teletherapy or External Beam Therapy (EBRT): When radiation is artificially generated outside the body and delivered to the tumor present inside the body then it is called outside External beam Radiation therapy (EBRT). 
2. Brachytherapy: Very miniature size radiation producing metals called isotopes (Iridium-192) are placed at the center of the tumor that slowly kills the tumor. This type of radiation is called brachytherapy or Internal Radiation therapy.

How radiation therapy is delivered: To control radiation and use it as per requirement, Radiation oncologist uses a very sophisticated machine called Radiotherapy Units. Eg.
Cobalt Unit (Oldest): One of oldest machines which was removed by most of the hospital and outdated now. This was able to do basic type of radiation therapy and was associated with a lot of toxicity and poor tumor control. 
Linear accelerator (also called LINAC): With technological advancement, scientists developed newer machines that can produce radiation only when desired and are computer controlled. These machines are called Linear accelerators or LINAC. Over the last few decades, lots of advancement happened in technology, computer. Few old or basic LINAC are Unique, Clinac and advanced Linac are Clinac iX, synergy-S and Tomotherapy. Latest and most advanced are True beam Linac.

It is “One in all Linac” that is designed to perform all types of available radiation therapy with high precision (less than 1mm accuracy). True beam can treat simple or complex cancer, static or moving cancer, single or multiple cancer, small or big cancer or even complex cancer present in the pediatric patients e.g. complete Craniospinal in Medulloblastoma or total marrow lymphoid irradiation (called TMLI or TBI). Most important feature of this machine is to perform Stereotactic radiation therapy such as frame based or frameless Stereotactic Radiosurgery (SRS) and Stereotactic Body Radiotherapy (SBRT). With use of Rapid arc technology, it is capable of fast (2 minutes per day) and precise (Sub-millimeter accuracy) Radiotherapy. 
With the TrueBeam system, treatment can be performed with unparalleled ease, precision and speed. These technologies make it possible to deliver fast, accurate image guided treatment within just a few minutes.
The linear accelerator is equipped with a beam shaper, On board imager (OBI-Kv), Orthogonal Kv X-ray imager, Cone Beam CT scanner (CBCT), 6D Robotic couch and Respiratory Gating (Real Time Position Management) system.

Which brachytherapy unit are available at SICMAR:
At Subharti Cancer Hospital, we have the latest 24 Channels HDR Brachytherapy Afterloader, model- GammaMed plus machine.

• Types of brachytherapy facilities available at SICMAR: Our oncologists are highly experienced in performing various types of brachytherapy. We at Subharti Cancer Hospital are only in the city to offer the state-of-the-art High Dose Rate Brachytherapy (HDR) Brachytherapy. The High Dose Rate (HDR) unit uses Iridium192 source to ensure a short treatment time e.g. in minutes rather than days as in the case of low dose rate radiotherapy (LDR). Brachytherapy requires insertion or placement of rigid or flexible hollow tubes inside or at the surface of the tumor. These tubes are connected with 24- channels of HDR brachytherapy units through which HDR brachytherapy sources travel to reach the center of cancer or post op bed and deliver radiation therapy. Based on the type of procedure, brachytherapy can be many types. We can perform the following types of Brachytherapy. 

• Mold / Surface Brachytherapy: When Brachytherapy applicators are placed in close proximity and over the cancer and they are called Surface or mold brachytherapy. It is used for superficial cancer e.g. Skin Cancer of external ear, Skin, Palate etc.
• Interstitial Brachytherapy: In this, multiple, thin hollow tubes are placed inside tumor or at post op tumor bed in parallel fashion. e.g. Carcinoma of Lip, Buccal mucosa, Eyelids, Tongue, Nodes, Breast, Lung, Liver, Sarcoma, Prostate, Cervix (MUPIT or perineal template)
• Intraluminal Brachytherapy: After delivery of EBRT, sometime we require escalating dose or boost to primary tumor located at luminal site E.g. Nasopharyngeal Cancer, Oesophageal Cancer, Cholangiocarcinoma, Vaginal Cancer (Sarbo), Anal Cancer etc. Such brachytherapy is called Intraluminal brachytherapy.
• Intracavitary Brachytherapy (ICRT): this is an integral part of radiation therapy for carcinoma Cervix. After completing EBRT, patients with carcinoma cervix and carcinoma endometrium need to undergo ICRT or ovoids brachytherapy. 
• Intraoperative Brachytherapy: When brachytherapy is performed in OT just after removal of tumor then it is called Intraoperative Brachytherapy. In this, needles or applicators are placed in post op beds especially in recurrent conditions where there is possibility of close or positive margin.  
• MR guided Brachytherapy: When we use MR compatible applicator and plan brachytherapy after taking MRI then these are MR guided Radiotherapy.

About Cancer Treatment:
Cancer is a big taboo for society. It can be prevented in 50% of case by raising awareness among local public to leave use of cancer-causing toxic materials like tobacco, arsenic, asbestosetc, wide spread use of timely vaccination for HPV, Hepatitis B etc., adopting healthy life style and intake of healthy diet. Cancer cells are abnormal cells within the body that grow in in uncontrolled and unprogrammed ways and forms ulcers or tumor that eighter damage or the organs where they live or can spread in surrounding structure or spread to any part of body.
Standard treatment for most of cancer are-

1. Surgery: In this, surgeon removecancerous tumour from your body and doctor known as surgical Oncologist or speciality surgeon specialist in organ based surgery like Thoracic Oncologist, Endocrine and breast oncologist, GI Oncologist and Uro-Oncologist etc.
2. Radiation therapy: It is most important treatment and almost all patient require Radiation therapy during his journey of cancer treatment either before or in between or later on. Such doctors are known as Radiation Oncologist. Radiation oncologist uses special types of very powerful ray that can kills cancer cells inside body. These rays are invisible e.g.  gamma rays or electron, proton, carbon particles etc.These rays are special as neither we can see, nor feel. and chemotherapy. 
3. Chemotherapy: like antibiotic that kills bacteria. Chemotherapy are cytotoxic medicines in the form of injections or tablets that kills cancer cells. Thesedoctors are called Medical Oncologist.

Cancer treatment in Todays era-

1. Multidisciplinary team approach:

Cancer treatment is evolving rapidly. Lots of development and advancement happened in all field eg.High end robotic True beam linear accelerator,multichannel HDR Computerized Brachytherapy in Radiation Oncology, Organ preservation surgery or Laparoscopic / Robotic surgeries and some Hormonal therapy, Targeted therapy, Immunotherapy in Medical Oncology. Now cancer treatment is expanded beyond stand above 3 specialties. That involve plastic surgeon, oro-maxillofacial surgeon for reconstructions as well asphysiotherapy / rehabilitateexpert for good functional outcome use of prosthesis for speech or movement. Sometime special cancer may be treated with special treatment done by Nuclear medicine colleague like Iodine therapy, Lu therapy, Sr therapy etc.

2. Personalised approach to incurable cancer: Some times are excising treatment is not able to give satisfactory result. For this situation, science have developed some special test e.g. liquid biopsy where we able to abnormality (called Mutations) presentin the cancer cells that can be destroyed by targeting these abnormality and ultimately destroy cancer cells.
3. Novel Therapy for Cancer-There is a lot of research work is going on genetic therapy where we able to correct abnormality present in gene so that we may able to cure some incurable paediatric cancer patient e. g. or Car T cell therapy

What to do if we have suspected for Cancer?
Whenever, your primary care or family doctor suspect that one may have cancer, he may do some routine blood test, special blood test called tumor marker (e.g. Ca 19.9, CEA, Ca 125, AFP, LDH etc), FNAC, Biopsy, CT scan, USG abdomen or may refer you to consult Oncologist (Surgical, Radiation or Medical) for diagnosis and confirmation. Once diagnosis is confirmed. Oncologist may advise CT scan or chest, USG abdomen or PET CT to know stage or spread of disease.
What is Tumour board?  Tumour board is a team of different specialty that are involved in management of cancer treatment e.g. Radiation Oncologist, Surgical Oncologist, Medical Oncologist, Specialty surgeon, Radiologist, Pathologist, genetic couseller etc. Every case is discussed in tumour board to decide plan of treatment, sequence of treatment and any special way to manage some unique case etc. this multi-modality approach is associated with best hange of cure and higher quality of life.
